CNA (Certified Nursing Assistant)
Our locally owned and operated home care agency Advanced Nursing and Home Support serves the greater Washington, DC metro area, including Maryland. We are growing and need to add to our team. We seek experienced, caring and professional CNAs who want to be apart of an amazing and caring team! We offer flexible hour to fit any schedule. Major Duties and Responsibilities include but are not limited to: Helps the patient to maintain good personal hygiene. Assists in maintaining a healthy, safe environment. Plans and prepares nutritious meals. Go to the market when instructed to do so by the RN. Assists the patient with ambulation. Assists with certain treatments as ordered by the physician and approved and supervised by the nurse. Assists the therapy personnel as needed with rehabilitative processes. Assists with medications ordinarily self-administered as assigned. Assists in household services essential to health care in the home. Taking and charting vital signs charting intake and output. Encourages the patient to become as independent as possible according to the nursing care plan. Attempts to promote patient s mental alertness through involvement in activities of interest. Gives simple emotional and psychological support to the patient and other members of the household. Establishes a relationship with patient and family which transmit trust and confidentiality. Prepared a daily report of her visit on the day it is performed and submits same in the clinical record weekly or as directed. Reports any change in the patient s mental or physical condition or in his home situation to the Clinical Director or the RN. Carries out the assignment as documented in the Care Plan and/or as instructed by the nurse or the therapist and reports to the Clinical Director when she is unable to do so. Works with personnel of other community agencies involved in the patient s care as directed by the Clinical Director. Performs routine housekeeping tasks are related to a safe and comfortable environment for the patient as instructed by the CD. Attends in-service as required by state and federal regulations. Confirming on a weekly basis, the scheduling of visits with the Coordinator/Clinical Director, to coordinate necessary visits with other personnel. Immediately notifies the Staffing Coordinator of any changes in the patient s schedule. The coordinator is responsible for notifying the patient/family. Notifying the agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings that will affect agreed service. Adheres to all Policies and Procedures of Advanced Nursing and Home Support Required Qualification: Certification: CPR and First Aid card Licensure: Valid CNA license. Experience: One Year of practical experience as a Certified Nursing Assistant in a home care setting. Major Duties and Responsibilities include but are not limited to: Participates in the planning and coordination of total patient care in conjunction with the RN and the physician s plan of treatment Follows the nursing care plan for each assigned patient Observes the patient; Evaluates the care of the patient; Applies her knowledge of nursing skills to the patients to whom he/she is assigned Accurately reports and records the patients condition and care, including signs and symptoms which may be indicative of a change Prepares clinical/progress notes on the day of the visit and incorporates same in the clinical/record/weekly (if applicable) Prepares summaries in cooperation with the RN supervisor Assists the patients with the activities of daily living while teaching appropriate self-care techniques Takes physician orders over the telephone Provides and maintains a safe environment for the patient. Provides an optimum physical and emotional environment Assists the physician and registered nurse in complex nursing situations Participates in case conference, in-services and other programs and meetings as needed Notifies agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings that will affect agreed serviced with the agency Perform treatments as assigned per physician s plan of care. An LPN may perform only those treatments for which they are qualified by training and experience to perform. If an LPN provides IV therapy or other technical procedures not included in routine LPN training courses, they must first show documentation of competency/experience signed by an RN, physician or specific training program director. Required Qualification: Certification: CPR and First Aid card Licensure: Valid, unrestricted, LPN license in the state of Maryland. Experience: One Year of practical experience as a Licensed Practical Nurse in a homecare setting. Vent experience not required but preferred.
