Under the direction and supervision of Chajinel Home Care Services, the In home Care Provider, provides direct services at client's homes. RESPONSIBILITIES: -Provide personal care and assistance at patient's residence, which will maintain and increase the ability of an older person to live safely and comfortably in their own homes. Tasks may include: homemaker service, assisting with ADL'S. Other tasks such as client's laundry, shopping, and monitoring vital signs. -Provide emotional and/or behavioral support to clients, for example to encourage clients to practice recreational activities, exercise, or social activities and remind them to follow through on nursing/medical care and diet restrictions. -Responsible for keeping areas of patient care clean and safe around the house. -Report progress and/or any other situation to the supervisor, keeping records of patient's progress and activities and any other important matter regarding their care. -Working for a program that provides services seven days a week requires working a flexible schedule, and ability to work weekends and on holidays. -Escorts and assist clients to, from and at medical appointments when need it. EDUCATION: -Valid California License as a Certified Nursing Assistant or equivalent 1 year of experience. -Department of Social Services ID number Preferred but not necessary. BACKGROUND AND EXPERIENCE: -Sincere interest in working with older people. -Ability to work as part of a team with flexibility, initiative and imagination. -Knowledge of the community served. -Bi-Lingual in English and Spanish, Cantonese, Russian or any other language highly preferred. -Interest and willingness to work in a multilingual, multicultural environment. ADDITIONAL REQUIREMENTS: -Able to lift and move up to 40 pounds Ability to understand and speak clear English -Working for the Aging community itself is a great opportunity to learn and make a difference in our community.
