RN Case Manager Positions: Scottsdale, North Phoenix, Cave Creek, Carefree Must be available to case manage M-F 9am-5 p.m. however visits are flexible to work around your life schedule. Visits are per-diem and flexible to work around your life schedule! The Registered Nurse Case Manager is responsible for managing, supervising, planning, teaching and providing nursing care to individuals and their families in the home. The Registered Nurse Case Manager works with other team members and the patient's care providers, and has overall responsibility for the planned program of care. The Start-of-Care/Re-Visit Registered Nurse is responsible for providing the initial home care assessment and/or re-visit with skilled nursing care and teaching to individuals and their families in their homes. The Start-of-Care/Re-Visit Nurse works with other Nightingale team members and the patient's care providers without providing Case Management for the patient. Requirements: Minimum 2-years Acute Care Nursing Experience Current Professional RN License Current CPR, TB, Fingerprint Card Exceptional Judgment/Decision Making Drive, Commitment & Initiative Data entry proficiency Ability to work independently Exceptional interpersonal and communication skills Adaptability & Creativity Benefits: Flexible Schedule $2000 New Hire Sign-On Bonus!
