About who needs care: Stage 4 kidney disease, chronic hypertension, polyneuropathy, diabetes, macular edema, high cholesterol, history of strokes, left sided weakness, Get up at a decent hour, shower, eat breakfast, do something fun, nap, lunch, PT/ot, nap, dinner in-between meds, About the care needs: Someone who is very patient, can keep structure and coddle at the same time, someone who listen and be very caring. My aunt loves kayaking, clamming, dancing, and going on adventures unfortunately can no longer do these at this time and needs companionship and help due to gait problem tends to have falls at times. She needs to be told to use walker and cane and also help manage meds Services needed include: transportation, companionship, and mobility assistance.
