OBJECTIVE: : To work with vulnerable children and families 1/20/2015-7/24/2015 Social Service Director Nursing Home At the nursing home I was responsible of doing assessments on resident for cognitive purpose. I investigated all abuse allegations. I was responsible for getting the grievances to the correct department and follow up until they were resolved. Some of the patients came in for rehab only. I was responsible for planning discharge meetings as well as referring patient to home health agencies. I also participated in all family care plan meetings. I further responsible for scheduling special care doctors to come to the facility. I was responsible for all behavior plans. Then there was just days that I had to do different things. The social services director had to attend all meeting. 8/1/2014-01/6/2014 Department of Juvenile Justice At the Department of Juvenile Justice I was responsible for a caseload of juvenile offenders to ensure they obeyed the orders set forth by the court. I made home visits with the families as well assisting families with finding services needed for drug treatment or other needed services. 9/13-12/13 Sales Associate There, I assisted customers with loan documents, finding furniture for their home, as well as customer service after the sale. I also assisted in marketing by taking sale ads to other businesses and to homes to assist customers and future customers with information about the merchandise that had in store. 4/13-6/13 Marion County Adjustment Center At the adjustment center I was an Addictions Treatment Counselor for approximately 50 male inmates with felony drug charges. I conducted assessments through the University of Kentucky's tracking system to score current inmates to evaluate the risks for re offending. I also completed assessments with each inmate in my unit to evaluate their needs, and risks as well as recognizing the need for professional intervention while incarcerated.
