Male, early 50's, suffered TBI 6 yrs ago. Lives in VSH home with social work support. Attends day program Tues -Fri. Issues with executive functioning, short term memory and initiative. Ambulatory. Left sided weakness and visual impairment. Highly articulate with great sense of humor. Is very caring and respectful. Enjoys history, trivia and movies. Needs a mature person who treads softly and does not try to micromanage. It would be very helpful if caregiver was already familiar with Moms in Motion/ Public Partnerships, since this care is approved under a Waiver. Some areas of assistance would be medication oversight, organization of living space, housekeeping, encouraging self care, healthy habits ( diet and exercise ), transportation and socialization.
