About who needs care: Need a retired LPN who can interpret vitals, ensure a healthy lunch, adherence to exercise and insulin regimen. Periodically, will need to fill pill box, grocery shop, accompany to doctor appointments (adjusting her meds as indicated) and take her to lab appointments. On rare occassions, may need to help her with technology (cable box reset, reset clocks, new hearing aids, etc). The patient has been hospitalized twice in the last month, has gotten confused from a UTI and began forgetting insulin. She can't go back to independent living without daily assistance until she is maintaining her Glucose herself. For the first few days, she may need a second daily visit until she is strong enough to pick up her dinner or walk to the dining hall. But she can take her own BP and has a Glucose Monitor, so morning and evening phone calls to interpret those vitals and evaluate or remind her on meals & pills can manage the rest of her day unless an emergency arises. About the care needs: Someone who: - is patient and can tolerate repeatedly explaining/teaching. - can recognize distress in a diabetic and symptoms of a UTI. - is comfortable contacting her doctor with pertinent information. - is helpful around the apartment and straightens up after her when she can't. - is focused on getting her back to independence with reduced help (ideally 1 or 2 in person days/week with daily calls) - will be a long term freindly, trusted advocate for her health Services needed include: transportation, meal preparation, errands / shopping, and light housekeeping.
