About who needs care: 94 yrs old male, with recent hospitalization due to SDH clot that rebled. Intermittent impaired memory, no Alzheimers. Enjoys taking walks but can only ambulate with a FWW x 100 feet with contact guard assist. Needs w/c for long distance. Likes to watch wrestling. He needs assistance with bathing, toiletting for safety. Hard of hearing, use voice talker device. Have been withdrawn since hospitalization/subacute rehabilitation x 100 days. About the care needs: Prefers male. Familiar with elderly care, patient transfer from bed/wheelchair, walking with patients, and assisting with ADLa/bathing/toileting. Services needed include: mobility assistance.
