About who needs care: On Oxygen, stand pivots to/from bedside commode and transport chair, recurrent pneumonia, frail. About the care needs: Intelligent, positive, encouraging, patient with providing assistance to allow patient to do as much as possible. Good communicator, good note taker for transfer of care to morning caregiver. Services needed include: companionship, and mobility assistance.
