Provide friendly, compassionate and supportive caregiving to help maintain independence. Able to provide companionship, as well as light housekeeping, meal prep, personal care, mobility and transfer assistance, transportation, medication assistance, and experienced hospice care. Hospice care includes the above as well as monitoring and keeping a log of patient's vital signs, medications, and food and water intake. Also monitor changes in behavior, mental or physical changes. Repositioning and monitoring for pain is performed regularly to maintaining comfort. Collaborate with hospice nurses to develop a specialized care plan for the needs of my client. Follow care plan and perform wound care as directed. Also able to support special dietary needs as they change.
