I have a degree in Geriatric Medical Social Work with extensive experience as a Medical Advocate and have owned my own private care coordinating business for over 20 years. I can accompany to medical providers and work with your providers on medication management and services needed to keep you or your loved one functioning at the best for as long as possible. Experience with discharge planning from facilities making the transition seamless and comfortable. Hospice is one of my passions as well. I offer concierge services accompanying you on trips, seasonal moves and the like. You or your loved one will be in qualified and experienced hands.
