About who needs care: Complex case with copd, chf, diabetic, fractured hip, anemia. Patient is a fall risk, uses a walker, uses bedside toilet and urinal. He sleeps most of the day but needs assistance standing, getting to the table to eat, preparing meal, toileting, taking his meds 3x a day, doing an albuterol treatment 1-2x daily. Patient enjoys playing cards and watching tv: he likes to read airplane magazines, sometimes can sit on the porch but gets cold easily. While sleeping he needs help to stay in a safe position keeping him in the bed. He tends to move around a lot in his sleep during lucid dream states. About the care needs: This patient is very easy going and easy to get along with. He is mostly compliant with anything but sometimes tries to be self sufficient and may need some gentle reminders of his physical condition . Provider needs to be able to help lift him out of the chair if need be. Must be able to assist with proper toileting hygiene. Must be engaging with the client when he is awake that is encouraging and uplifting never expecting him to do more then his capacity allows. Must have a good reputation or rating with past clients or other referrals. Must be able to assist with light house work on the 1st floor where patient is located. Must have quality communication skills to both client and family. Services needed include: mobility assistance.
