Field Supervisor Job Description 04/04/2024 The purpose of this position is to provide the below services in the client's home that he/she would perform if they were physically capable. The intent is to enable the individual to remain safely and comfortably in their own home, or primary residence. Field Supervisors: The right candidate for this position must have weekend and evening availability. Must have an active CNA License in DORA. Must have complete knowledge of all IHSS (Health Maintenance) clients. Be able to train new care providers, on all health maintenance tasks. Field Supervisors must work together to meet the needs of all IHSS clients. Take on call once a month. Meet regularly to discuss issues in the field primarily associated with IHSS clients. Review client notes in clients' homes, making sure they are complete professional and well documented. Assist with medical management, appointments, and medication assistance. Perform supervisory visits as time allows. Field Supervisors must be willing to: Assist clients with Health Maintenance activities such as transferring using adaptive equipment, assist with ostomy and catheter care, wound care, respiratory care, medical management, exercise, and mobility. . Assist clients with Homemaking, encompassing light housekeeping, meal preparation, laundry, and shopping. Assist clients with Personal Care including but not limited to bathing, dressing, eating, transferring, walking, toileting, continence care, medication reminders, hygiene assistance, as well as Homemaking tasks. Candidates must exemplify our Core Values which are Integrity, Dependability, Adaptability, Communication and Free Thinking. Candidates must be able to submit accurate documentation encompassing time tracking and progress notes. Be able to read and follow a clients Care Plan. Must be dependable and capable of pleasantly and tactfully communicating with clients, family, staff, and other agencies.
