The part-time Nurse Manager/Clinical Educator is a Registered Nurse (RN) who is responsible for the overall direction of home health clinical services. You will have overall responsibility for the quality, adequacy of services provided and supervision of staff (including CNAs, LPNs and other RNs). You will establish, implement, and evaluate goals and objectives for home health services that meet and promote the standards of quality. We are looking to add an enthusiastic and talented leader to our agency which serves primarily of the communities of Plymouth, Minnetonka, Wayzata, Orono, Medina, Mound, Delano, Rogers, St. Michael and Buffalo. $33 to $35 per hour
Weekly pay with direct deposit
Provides care as defined within the scope of the state s nurse practice act.
Makes the initial evaluation visit, complete the care planning process and regularly reevaluates the client s nursing needs.
Accountable for internal case management.
Prepares clinical and progress notes and summaries of care.
Assists in the development of organization goals. Develops, recommends, administers and insures adherence to agency policies and procedures.
Leads field staff in delivering exceptional client service and quality clinical care.
Supervises, educates and coaches other nursing personnel/care-giving field staff.
Plans and implements in-service and training programs, including orientation sessions for new field staff (every other Thursday afternoon to start).
Provides clinical insight and focus on clinical competency of your staff.
Oversees the maintenance of client clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
Assures proper maintenance of clinical records in compliance with local, state and federal laws.
Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel.
Promotes home health referrals in the health care community.
Current unrestricted RN license in Minnesota.
Preferred: Bachelor s degree in Nursing. Must have graduated from a National League of Nursing accredited program
Minimum one (1) year of documented experience required. Three (3) to five (5) years of clinical experience with one (1) year experience in a home health agency, hospice or health facility.
