Please see the full Licensed Vocational Nurse Job Description below: RN Job Description Purpose: To Make Lives Better, one visit at a time Objective: To provide professional LVN services and enable the client to remain independent in their own home. Job Summary: Delivers client care and services on case by case basis in accordance with the LVN standards and Nurse Next Door s policies. The LVN works with the client, their families, the Care Designer and the health authority, supports and makes recommendations for improved systems and processes and continuously assesses the effectiveness of care delivery in support of achieving the clients goals. Qualifications: Current Certificate of Competence and in good standing with the provincial College of Nurses Preferred: Bachelor s degree in Nursing. Must have graduated from a nursing accredited program. Minimum two (2) years of clinical experience with at least half of that time working in home health or related health care organization. Have a smart phone. Have a valid driver license and auto insurance. Be willing to work with clients that have pets in home. Possess exceptional nursing assessment skills Comply with accepted professional standards of practice. Proven ability to build quality and implement improvements within the operation. Excellent interpersonal skills. Analytic/Critical thinker. Excellent customer service skills and ability to deal tactfully with clients, staff and community. Treat clients, employees and the public with courtesy, respect and presents a positive public image. Participates in decision-making relating to matters of health and safety, client care, and human resource management. Ensure that the Care Designers, clients family and Care Services Center are updated about the changing needs of the client. Implementation of changes in care delivery resulting from doctor/Care Designer s decision-making. Delivers comprehensive professional nursing care consistent with the caring plan, nursing assessments and client needs. Advises the Care Designer of trends, changing needs and precedents that impact the effectiveness of care and service delivery and warrant adjustments to the program. Ensures the client right to privacy, dignity and respect are consistently met throughout care and service delivery. Advocates for clients and supports families in maintaining meaningful relationships with their loved one. Communicates with client and family members concerning care issues, needs, problems and/or conflicts. Effects positive outcomes to complaints and reports unresolved issues to the Care Designer. Ensures proper maintenance of clinical records in compliance with provincial and federal laws. Ensures that proper procedures are followed in the handling and storage of confidential client, team members and/or company information and medications. Ensures adequate supplies are available to fulfill program requirements. Provides care as defined within the scope of nursing practice and the Regulated Health Professions Act. Functions within the limits of own experience and knowledge, and practices safely and competently within the job description. If directed by the Care Designer, makes the initial evaluation visit and regularly reevaluates the client s nursing needs. Provides those services requiring substantial and specialized nursing skill. Initiates appropriate preventive and rehabilitative nursing procedures. Informs the Care Designer, physician and other personnel of changes in the patient s condition and needs. Identifies and participates in Nurse Next Door performance improvement activities. Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel. Provides companionship to the client by reading, talking, listening and performing related psychosocial activities. Assists clients in all activities of daily living/hygiene assistance, bathing, grooming, linen changes and so forth. Prepares delicious meals within the patient s diet, assist client with eating and follow up with clean up. Assists clients with transfers, ambulation and exercises. Performs housekeeping chores that the client is unable to perform including vacuuming, make beds, laundry, dusting, floor washing, cleaning the bathroom and kitchen. Assists client in the bathroom or with use of a bedpan. Answers clients calls and attend to their requests promptly and positivere Designer on call IMMEDIATELY. Communicates availability to work to the Care Services Center as required. Maintains confidentiality regarding the client s condition and his/her family, with the exception of Nurse Next Door Staff and client s physician. Meets the safety needs of the client.
