About Great Clinical Services

Our practice is dedicated to providing in home services to the growing population of adults who are 65+ and older and their network of caregivers. We will work with clients and families to identify levels of functioning, address mental health issues such as grief, depression, and anxiety, and identify barriers and strengths in both the home and family environment. Our goal is that all clients should remain as independent as possible, SAFELY, in their own home environments.

SERVICES
An initial phoneconsultation (no charge) will be conducted to gather information and assess needs to determine if our clinical team is an appropriate match for your needs and goals. <br/> <br/>Intake and Assessment: In home initial intakes will take approximately 1.5-2 hours. We will obtain all relevant history and obtain appropriate collateral information from caregivers. We will sign release of information for the community providers if needed. <br/> <br/>Further services may include: Individual Therapy to address depression, anxiety, grief, and loss. Family Therapy with or without the client as needed to address long term planning. Case Management and referral as needed. <br/> <br/>We offer comprehensive clinical case management.
