#1. CARE-GIVING: services are provided by home health aides, companions and certified home health aides.
#2. Chronic Illness & Symptom Management: These services are designed to reduce frequent trips to the hospital and rehab facilities. The care programs are designed by nurses and geriatric care specialist.
#3. CARE MANAGEMENT & CARE COORDINATION: These services are provided by geriatric care managers. The goal is to make long distance care giving easier and more gratifying for those living long distances or the very busy family caregiver.
#4. Mobility, TRANSFER, and functional fitness to speed up recovery after a hospital or rehab stay.
