#2. Chronic Illness & Symptom Management: These services are designed to reduce frequent trips to the hospital and rehab facilities. The care programs are designed by nurses and geriatric care specialist.
#3. CARE MANAGEMENT & CARE COORDINATION: These services are provided by geriatric care managers. The goal is to make long distance care giving easier and more gratifying for those living long distances or the very busy family caregiver.

My father in law passed away in July. Heidi provided the home care needed for a little over a week. She was our main care giver. She was phenomenal! So gentle and caring with my father in law and all of the family members who were there at the house during that awful time. She administered medication, kept vitals, bathed, fed and just spent time with him talking. Her manner was so loving even though she only knew him for a little over a week. She was so helpful too myself and my husband while we stayed at the house. She is very skilled in her abilities. I consider her an angel sent from god to help take care of him until he was ready to pass and to help our family as well. She is my hero and angel. I recommend her whole heartedly. You will not be disappointed.
