As an LPN, supporting the mission of Wesley Community will include the following responsibilities: Serve as a Medication Nurse and/or Team Leader, Perform and direct resident care in accordance with current federal, state, and local standards, guidelines and regulations that govern our facility and as planned by the Unit Coordinator Ensure the highest degree of quality care is always maintained. Cooperate with other resident services to ensure the continuity of the residents' total regimen of care is maintained. WHAT IS REQUIRED OF ME FOR THIS ROLE? As an HHA, supporting the mission of Wesley Community will include the following responsibilities: Provide health care tasks, personal hygiene services, housekeeping tasks, and other related support services essential to the client's health. Record and report changes of the client's physical condition, behavior, or appearance to the Director of Clinical Services. Perform light housekeeping duties including dusting, vacuuming, mopping, & sweeping. Assist with bathing, dressing and grooming. Assist with transfer and ambulation including the use of a cane, walker, and wheelchair. Assist client with miscellaneous household duties. WHAT IS REQUIRED OF ME FOR THIS ROLE? Must have high school diploma or GED equivalent. Must have successfully completed a training program in home health aide services and earned a certificate from an approved Home Health Aide Training Program. Must be 18 years of age. Must have your own reliable transportation to get to and from clients homes and occasionally transport clients. Must have a current and valid Driver's License in the state of New York. Must have clean driving background and will be subject to yearly background checks.
