About who needs care: I am a very active woman with severe Cerebral Palsy from Willoughby in Lake County. I live in my own condo with my service dog and cat. I work full-time for Services for Independent Living, Inc. in Euclid as the PCA Program Contractor Supervisor.
I'm going to be honest--my independent provider who worked for me for 24 years passed away in April. I am extremely heartbroken. I found someone to work some of her hours, but I still need a provider or providers to work the rest of the hours.
The hours I have available are:
Tuesdays 6:30am-6:30 p.m.
Thursdays 6:30am-6:30 p.m.
I work some days remote from home and some days in the office. I work from 8:30 a.m. to 5:00 and this is why I need 12 hours.
I need someone who is willing to work for a home health agency or become an independent provider under Ohio Medicaid because I have a Medicaid Waiver.
General information about me:
-No MyCare Ohio--straight Medicaid; CareSource is the case management agency
-Power wheelchair user
-Need help with all ADLs.
-Have a speech impediment.
-Highly intelligent (masters degree in rehabilitation counseling)
-Need someone reliable, dependable, and cheerful.
-I have a modified van that you would need to drive.
-Must be okay with a dog and a cat.

About the care needs: Must have good driving record and be reliable and dependable.
Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
