Responsibilities: * Care Assistant CNA performs duties as assigned by Care Manager including ADL baths, oral hygiene, foot care, skin care, pressure relief, dressing the client, feeding, transfers, ambulation, exercises and IADL support. * Care Assistant Companions performs non-direct support care duties as assigned by Care Manager * Demonstrates Initiative * Ability to cook and prepare healthy meals [Mediterranean diet] * Describe activities to promote social and mental engagement and Implement planned strategies to assist the client. * Employ proper body alignment to optimize client and assistants' safety. * Supports and maintains a culture of safety and quality. * Identifies and communicates findings to the Care Manager or designated Supervisor. * Demonstrates the ability to chart according to professional standards. * Completes all necessary documentation. * Works as a team player.
