Geriatric Care Manager navigates the elder care system. A Geriatric Care Manager coordinates care and facilitates communication between family members, doctors and other providers in order to maximize the older adults quality of life.
Geriatric Care Management services include: Comprehensive Care Plan assessment in clients home, hospital or facility - Initial consultation fee $150
Initial consultation will include an assessment and identification of client's needs, concerns and preferences including:

Development of Care Plan that includes:

Gretchen graduated with a Masters in Gerontology from Concordia University in River Forest, Illinois. Before founding The Bridge, she worked with the Oak Park Township Senior Services. She has also worked for the Department of Aging and the Kenneth Young Center (KYC) in Elk Grove Village, Illinois. Gretchen has held two positions at the KYC, first as a Choices for Care Coordinator at St. Alexius Hospital in Hoffman Estates, and most recently as a Care Coordinator through the Department of Aging Community Care Program. Gretchen has been certified through the National Academy of Certified Care Managers.
