Geriatric Care Manager navigates the elder care system. A Geriatric Care Manager coordinates care and facilitates communication between family members, doctors and other providers in order to maximize the older adults quality of life.
Geriatric Care Management services include: Comprehensive Care Plan assessment in clients home, hospital or facility - Initial consultation fee $150
Initial consultation will include an assessment and identification of client's needs, concerns and preferencesincluding:
<br/>demographics, health history, social history, legal/financial history, medication assessment, fall risk assessment, nutritional assessment, depression assessment, home safety assessment, pain assessment, cognitive assessment and ambulation assessment.
