Sustained Hope offers specialized, technology-enabled home care services to those challenged with daily living because of age-related neurological diseases, medical trauma and other forms of disabilities.
We are not a me-too company. Unlike many franchises offering run-of-the mill services, we work with you and your guardian to set specific goals and put together a care plan to accomplish them. Our caregivers are cherry-picked and matched with the client needs.
We run complete background checks on ourcaregivers and our company is fully bonded and insured. We have several programs to keep the caregivers motivated to work for you and us--these include 3-phase orientation, continued education, additional certifications and on-the-job rewards and recognition programs.
Our services include: hospital-to-home transition (unique and innovative), daily living personal care, companionship, rehabilitation service assistance and coordination, transportation and hospice care.
