About who needs care: Frank is 81 y. o. who recently had small strokes. He still lives in his own apartment but needs assistance with his medications, diabetic management, visits to PT and doctors, simple household duties, grocery shopping, getting dressed, and he has some skin care issues. While in the apartment, Frank ambulates without assistance but outside, Frank requires rollator walker/wheelchair. Along with the strokes, Frank has heart and kidney disease. Frank likes to talk about his days growing up, his time in the Army, jobs, friends, etc. and he likes to watch TV. Frank mainly needs someone to assist his AM routine, visits to PT/doctor/grocery store and his PM routine while I, his daughter, caregiver, and POA visit my in-laws in Florida.
About the care needs: Frank is quiet but has a good sense of humor. Need to be able to check blood sugars, manage insulin, medications, fit a rollator in vehicle, provide good skin care (Frank has fungal rash in skin folds). He likes someone to sit with him and listen to his stories.
Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
