Trans-Care provides skilled nursing, therapy, personal care and activities of daily living from one integrated team lead by a RN. Trans-Care promotes independence, keeps families together, the practice philosophy is to treat and support every client mind body and soul for fast recovery. We prevent unnecessary hospitalizations by teaching our clients disease management for self-sufficiency, implementing a fall reduction plan for home safety and performing medication management on every visit.
Trans-Care Home Health, is committed to providing a level of quality care that encourages the safe and cost-effective delivery of home health services, maximizing patient autonomy, and coordination of community and Agency resources, to decrease illness burden and unnecessary duplication of services, thereby promoting positive patient outcome.
