Sacred Care provides care coordination and care management by integrating a set of activities intended to improve the client quality of life, working together with client healthcare team & family to achieve healthcare goals and monitoring & evaluating the care delivered by our staff.

For many families it is difficult to keep a consistent caregiver, whether it is lack of skill, unreliable or achange of schedule. In result of an inconsistent caregiver the family and client suffers. In those times it leaves the client without care forcing a family member to provide care for their love one which also presents challenges for both parties.
