ABOUT ADULT DAY CARE SERVICES

Adult Day Service is a support program for caregivers, providing a safe haven for your loved one while you work, handle personal affairs, or simply relax. These services may also help someone who wants to remain in the community but cannot be home alone during the day.

Our Adult Day Program as an invaluable resource to assist you in meeting the challenges of being a caregiver and balancing this role with taking time to care for your own needs. With our support, you can:

*Re-energize yourself and maintain your own health and wellness so that you can continue to provide the excellent care that you have been providing. * Complete your "to-do" list and get caught up again. Our mission is to be able to provide direct care and supervision of adults in a community based setting. We hope to delay institutionalization in a nursing home or assisted living facility by providing a cost-effective alternative. We strive to promote social, physical, and emotional well-being for our participants while providing you with the peace of mind that your loved one is safe. SERVICES OFFERED Community Care Systems offers the following Adult Day Services: * Handicap accessible transportation to and from our facility * Nutritious hot meals and snacks * Medication set up and distribution, blood pressure and weight monitoring performed by both a Licensed Practical Nurse and Registered Nurse * Personal care such as bathing, shampooing hair, skin and nail care * Socialization among peers * Exercise * Intergenerational activities * Personalized Care plans * Caregiver Support Services * Emergency Respite We also offer Medication Set Up as a separate service for those not wanting to participate in the Adult Day Program.
