CNS Provides the following supports to our clients ranging from 2 to 104 years of age Community Living Supports, Personal Care, Homemaking, Respite and more! Included is the community inclusion/integration- one- on -one supports to an individual out in the community to achieve independence using his/her natural resources, assisting with activities of daily living and personal care including, but not limited to: bathing, mouth care, skin care, shaving, dressing, feeding, positioning, transferring, ambulation, exercise, toileting. Other duties could include assistance with housekeeping, meal preparation, transportation, escort to medical and other appointments, companionship, and respite. Staff's documentation is mandatory in following each individuals plan of care. Problem solving skills are necessary since working independently.
