The care is personalized to suit your situation from one hour to 24 hours of care, 7 days a week, all year long. <br/><br/>#2: POST HOSITAL, POST REHAB CARE: <br/>This service is best for those who have lost some or all of the ability to care for themselves dues to medical incident. Our therapist and nurse will design a care program to help your loved one reach &amp; maintain the highest optimal &amp; functional level possible in the home. <br/><br/>#3. CHRONIC ILLNESS MANAGEMENT: <br/>These services are designed to reduce frequent trips to the hospital &amp; rehab facilities. The care programs are designed by therapist, nurses &amp; geriatric care specialists. <br/><br/>#4. CARE MANAGEMENT &amp; CARE COORDINATION: <br/>These services are provided by geriatric care managers.
