HOME DIALYSIS SUPPORT: These services are designed to give the dialysis patient a choice to treatments at home on the schedule they choose and have life according to them. These services are offered by nurses and dialysis technicians. We are available to do as little or as much dialysis care as you prefer. <br/><br/>#3. CARE MANAGEMENT, CARE COORDINATION &amp; PATIENT/FAMILY EDUCATION: We will help you simplify the complex medical information down to usable information that can transform your caregiving situation.
