These include: <br/> <br/>-Hospital Re-Admission Prevention Program- Our staff is taught to monitor vitals, treatment plans and medication adherence in order to prevent your loved one from going back to the hospital. <br/> <br/>-Fall Prevention Program- Our staff is taught to locate fall hazards as well as closely monitor your loved one's mobility so that we can actively plan and prevent your loved one from falling. <br/> <br/>-Suicide/PTSD Surveillance Program- Our staff is taught to recognize the warning signs of suicide and post-traumatic stress syndrome and how best to address these conditions. This program is specially tailored for veterans and patients who have experienced traumatic circumstances. <br/> <br/>-Cognitive Engagement Program- Our staff is taught how to engage clients in meaningful activities that help to improve cognition, memory, and socialization skills.
