About Daily Grace Adult Day Health Services

Daily Grace Adult Day Health Services is a community-based medical facility that provides socialization, health education, and rehabilitative care to disabled adults and seniors with chronic or acute medical illness, and also may have cognitive and functional impairments. Clients do not have to give up their home or move away from their family to participate in our day program. Participants engage in therapeutic activities, arts and crafts, music activities, indoor and outdoor games, gardening, andexercises.<br/><br/> Individuals who are socially isolated, frail, and are at risk for nursing home placement, someone who is returning to the community from a nursing home, rehabilitation facility or hospital, and needs help recovering to achieve optimum health and function are appropriate clients at Daily Grace ADH Services. <br/> <br/>Family caregivers who work, attend school, or have a busy schedule and have concerns about the well-being and safety of their care receiver would benefit from utilizing our services.
