About Forget Me Not Alzheimer's Life Enrichment Program Llc
We provide one on one & independent activity programs for seniors living with Alzheimer s and Dementia.
We also provide Non-Medical personal care as well as respite care services.
After the initial assessment is completed by the family & client, we conduct an informal assessment with the client through observation, background information, as well as physical, mental, social, emotional and leisure status.
After these assessment are completed, we will create an activity program for the client.
