About who needs care: 91-year-old female, history of dementia. She is alert but not always oriented to time, place or situation. She needs regular reminding. She weighs about 110-113lbs. Very limited in mobility, non-self ambulatory. needs assistance from bed to commode/ wheelchair. She needs personal care and good interactions. She often sleeps in until late mornings. Biggest challenge is getting her to eat her meals. About the care needs: Compassionate caregiver to assist with activities of daily living, companionship, emotional support , meals, personal care and medication management. Services needed include: companionship, feeding, bathing / dressing, and mobility assistance.
