About who needs care: She is a hospice patient with copd, chf, failing atrial valve. She is mostly incontinent and is experiencing an accelerating rate of decline. She is frequently confused. Hospice has reccomended 24 hour care. They are providing 1 daily aid visit M-F and a weekly nurse visit. About the care needs: My father is also in the home. He is 90, cognitively impaired, peripheral artery disease, chf, low back issues. He can take care of his own personal needs. Judgement can be questionable. Services needed include: meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
