About who needs care: Diabetes type 1 and 2 Chf Apnea Hypertension Hypercholestemia Liver chirosis type a I believe stage 3 , so reversable She has mobility issues Some cognitive depreciation. Can walk with assistance, but needs help as she is weak and needs a walker to get around. Afraid of falling. Needs help bathing Needs reminders like to take medications, on omnipod and sensor to monitor and deliver bolus insulin, but needs help remembering to deliver the bolus injection and possibly with applying the sensor and pod on. I come over to help with these tasks usually but cannot be available 24/7 as I work the weekends nights and teach classes and such throughout the week. Needs reminders to eat and hydrate Needs to exercise if possible with walking and getting up out of chair, leg exercises as tolerated. If someone could come for an hour or so at least a couple times of week to check in on her and get her back to where she needs to be that would be great. She is currently trying to lose weight and doing a good job with that, but the problem is she has not been eating this week. Not taking her insulin, because no one there knows how to do her pods correctly except me. My dad is overwhelmed and does not understand how to do it. And the technology behind it. And she is not being compliant with her lactulose. So I need to get her to the doctor at some point to discuss her medications and being compliant with it. About the care needs: Someone who is easy going, good natures and helpful. Gives here reminders and patient. Just needs help with daily tasks, medication reminders, eating well and hydrating. Maybe get her active again. She likes puzzles, shopping, watching TV, and such. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
