Rheumatoid Arthritis Right Carpal Tunnel Syndrome hypertension Atrial Fibrillation Arthritis of right knee Anxiety Total left knee replacement Lumbar Radiculitis Chronic low back pain with lumbar radiculopathy Prothrombotic c state Chronic pain Chronic neck pain Hobbies / Interests: Reading, watching certain movies, digital games, Eating out occasionally, drives to the countryside or beach area Ideal day: Assistance with ADL activities, light house cleaning, laundry, some shopping and/ or running errands when needed. Doctor visits and /or lab work that prevent me from driving myself. For example after receiving a facet joint injection, a driver is required to take me home. Having a companion when I drive myself. About the care needs: Intelligent communication, can communicate clearly, patient, knowledge in assisting a person who is prone to fall., mainly standby assistance, good hygiene, punctual. Services needed include: meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
