We are looking for a compassionate, experienced & licensed California RN to provide regular health monitoring & medical support for our Mom, age 80, who lives at home in La Jolla. Responsibilities: - Conduct weekly in-home health assessments (vitals, medication review, overall wellness check) - Accompany to all doctor appointments and medical procedures (Needs reliable transportation) - Take detailed notes during appointments and ask appropriate medical questions - Communicate with family members after each visit/appointment via phone or email - Monitor medications and ensure compliance with doctor's orders - Coordinate between different specialists and primary care physician - Alert family to any changes in condition or concerns - Maintain simple health log for family reference - Help interpret medical information and treatment plans
