Patient: 62-year-old male quadriplegic Location: Condominium in Chandler, Arizona Duration of Need: Daily assistance for approximately 1 month Reason for Care: Recently hospitalized for 6 days due to an infection, resulting in significant strength loss. Patient is currently unable to perform safe transfers independently and requires assistance for daily activities. Care Needs: • Assist with toilet transfers (bowel care required, easily taught) • Assist with shower chair transfers • Laundry: wash, dry, fold, put away • Light housekeeping and errands (groceries, pharmacy, misc.) • Occasional transport to appointments (accessible vehicle provided; no personal car needed)
