Client (87) former farmer and professor of plant genetics s. He lives with his wife who the same age. She is unable to provide his care without assistance due to declining health. He is starting hospice . He is nursing home level of care but wishes to remain in the home. Had a stroke 7 years ago, has expressive and receptive aphasia. He is short of breath with exertion. He uses a walker and requires reminders to use safely. Requires assistance for getting into and out of car. Requires assistance for shopping, bathing and dressing. meal prep , light housekeeping and companionship. He is loosing his eyesight and would like to be read to . Reading, riding in car to look at farms Basic personal care giver training. Must be able to transfer person into shower/ bath Driveway and street parking . No pets
