About who needs care: -home dialysis -has CHF - has become weak and needs support while I am at work. -have 2 dogs -his day is having breakfast, watching t. v., possibly napping. He is looking to be involved in an activity to keep his mind sharp. About the care needs: - carry on conversations with him - help with cleaning such as dishes, vacuum, his bathroom. -encourage him to do his physical therapy exercises Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, companionship, feeding, and bathing / dressing.
