Seeking Senior Care Provider In Farmingdale

I am my dad's sole caregiver. He's 63, young at heart but physically and medically, needs help. He's been in the hospital for the last two months and is now finally coming home. The hospital has arranged home PT, OT, and VNA but unsure of the schedule. I am going away from Nov 5-10 and need to feel safe and confident that my dad will be okay and watched when I leave. He is 6'3 and 230 lbs so definitely not little. He has dialysis three times a week and needs transportation. I need senior care provider with experience. Our care concerns are: Mobility Assistance, Wound Care, Help Staying Physically Active, Bathing / Dressing, Heavy Lifting, Transportation. One of your responsibilities will be helping with bathing. We ask that you handle transportation as needed. I can add more details and info if proceeded.
