Assist clients with personal care including bathing, dressing, grooming, toileting, oral hygiene. Provide mobility and transfer assistance, including help with walking, use of assistive devices, repositioning. Support meal preparation, feeding, light housekeeping (laundry, dishes, tidying up). Offer companionship through conversation, social interaction, emotional support. Observe and report changes in the client's physical or emotional condition to the supervisor or nurse. Follow the client's individualized care plan, maintain accurate documentation of services provided. Ensure infection control safety procedures are followed at all times. Accompany clients to medical appointments or errands, if approved.
