About who needs care: This would be for a medical appointment with a pulmonologist in Elgin Illinois. She is presently in an assisted living facility in West Dundee Illinois. She would need transport to and from, and assistance getting her physically in the building and into the doctor's office. She has limited mobility and requires a wheel chair. About the care needs: Transportation to and from and in and out of the doctor's office. Just a kind natured individual to work with my mom. Services needed include: transportation.
