I need adult care provider with experience. Our care concerns are: Light Housekeeping, Mobility Assistance, Help Staying Physically Active, Bathing / Dressing, Meal Preparation, Errands / Shopping, Transportation. We would prefer someone who can handle light housekeeping. Assistance with bathing is one of our requirements. We're looking for some help preparing meals. We might ask you to run errands now and then. We ask that you handle transportation as needed. Patient: Terry Hord Diagnosis: Advanced Multiple Sclerosis - Quadriplegia Mobility: • Requires Hoyer lift for all transfers between bed and wheelchair • Uses motorized wheelchairs for mobility • Has a handicap-accessible van requiring a caregiver/driver for medical transport Activities of Daily Living (ADLs): • Full assistance with bathing, dressing, and grooming • Hair wash
