He has been hospitalized or in rehab since March- last discharge August? Wound nurse comes 2x's weekly, PT x's weekly(only 1/2 hour session) Needs to be encouraged to do more self-care. Services needed include: meal preparation, errands / shopping, light housekeeping, and mobility assistance.
