About who needs care: John sits in wheelchair, cannot walk. I need help 3 times per day, bathing, diaper, plus bed transfer morning and evening. Weighs 144 lbs. coming out of rehab center soon. Need a wheelchair for home. Fractured hip and operated on 3 months ago. Starting with dementia and Parkinsons. Talks softly. Cannot walk, but rehab wants him up in wheelchair all day. About the care needs: A smile. He sleeps a lot because of medicine. He gets one hour of therapy per day in rehab. There will be a nurse checkup twice per week and physical therapy twice per week. Services needed include: transportation, feeding, bathing / dressing, and mobility assistance.
