We asses client needs, create care plans, coordinate and monitor services, and schedule and attend doctorappointments. > Medication Management - a registered nurse will set up medication containers weekly or bi-weekly and document the set up. The nurse will call the pharmacy to refill prescriptions. > Personal Care - a CNA will assist with activities of daily living including eating, toileting, bathing, dressing, grooming, walking and transferring. A CNA can also assist with light housekeeping and laundry, meal preparation, errands, transportation and be available for companionship.
