My name is Tara . I am an experienced healthcare social worker. I have experience working in hospitals, skilled nursing facilities, memory care units, and doctor's offices. My graduate degree is from University of Kansas. I am a certified Dementia Practitioner and a mobile notary in Ohio. I offer consulting for those caring for an aging loved-one with changing care needs. I provide in-home assessment, education, connection with support resources and a written, step-by-step plan for both immediate and long term care needs. In addition to consults, I'm offering community Care Management. This service is tailored to specific needs, but can include check-ins, medication organization/reminders, transportation arrangement, mail sorting, bill payment, coordination of meals and caregivers, socialization and more. I do not provide hands-on care, but I can make and manage all arrangements for it.
