Independent Patient Advocate & End-of-Life Family Consultant I have worked as an RN in various positions since 2000, ranging from acute care at MGH to hospice houses. My nursing training and experiences are with both adults and geriatrics. My passion and areas of focus are palliative and hospice care. I have acquired advanced education (masters in nursing) and worked as an APRN caring for hospice and palliative patients. Although I hold nursing licensures both as an RN and APRN, will not be operating in an official nursing capacity. I can provide you with my educated observation of changes, translate jargon, and function as a communication facilitator. I can also assist to ensure current comfort plan matches current presentation. During my highly individualized time spent I can review care documentation and track any gaps in care. We hired Tara to help with my dad who had a stroke. He was in a nursing center but we felt that he needed extra attention as we could not be there all day. Tara would spend a few hours with him each day and the time increased as he declined. She was a great advocate for him and a great communicator with us. She helped bridge the gap of what we wanted and what medical providers were doing until he passed. We can't say enough about her and how she helped our family through a difficult time.
