I have been a patient care coordinator for the last 10 years for several elderly people, up until the day they passed. I hired and replaced staff, filled in for staff, provided all supplies, groceries and transportation to and from doctors appointments, hospital visits and medical procedures,. I keep the family apprised when they cannot be available and try to meet every need of my patient and see them 2 to 3 times a week. I make all the doctors appointments and cancel when needed and deal with any facility administrators as needed. I'm usually on the patients HIPAA paperwork so the facility will deal with me. In addition to providing transportation and shopping for supplies, picking up mail, I cook meals when needed, do a little light housekeeping and a lot of listening.
